IV. OFFICIAL TRANSCRIPT

An official copy of your high school transcript must be included with your application materials.

V. FINANCIAL NEED

Please be advised that financial need is a factor in the selection of the scholarship.

Provide any relevant information you would have the committee review, if any, in evaluating this component of your application.

VI. REFERENCES

You must submit two letters of reference: one from someone familiar with your academic background,

and one from someone who knows vour work and service in the Portuguese community in which you are involved.

VII. INTERVIEW

Those applicants chosen as finalists will be required to attend a formal interview with the selection committee.

STATEMENT OF CERTIFICATION AND RELEASE

All of the information provided herein is complete and accurate to the best of my knowledge. I hereby give the American Foundation for Charities of Portugal Scholarship Committee permission to share this information among committee members for the purpose of evaluating and possible bestowal of the AFCP Scholarship.

I further certify that I am of Portuguese descent, and that I am a citizen or permanent resident of the United States of America. I also affirm that if I am given the award, I will become an active member of the AFCP Scholarship Committee for the 2003-2004 academic year.

I authorize mv high school to release pertinent information to the AFCP Scholarship Committee. I am aware that falsification of information will resuilt in the termination of anv scholarship granted, and that all application materials will become the property of the AFCP Scholarship Committee.
